What
is your responsibility in the ministry of formation and incorporation of new
members
in your community?
When did you/will you begin?
Do you currently have new members in initial formation?
What
possibilities do you foresee for meeting the program requirements of forming a
study
group with members of your own community and meeting regularly with a
mentor in the
context of a peer group?
Briefly describe those whom you are/will be accompanying, i.e. in terms of their life
experience, educational background, cultural traditions...
Write a brief essay in which you describe your own gifts, needs and hopes in relation
to the primary aim of the program, which is:
"The primary aim of ForMission is to foster the personal transformation
of the directors, and offer them help in developing the vision and skills
necessary for formation for a prophetic way of life."
How and when to apply:
Before applying, please contact the program director to arrange for a phone conversation
about the program and to determine space availability. The deadlines for sending the
written application are November 1 to begin the program with the winter session, and
April 1 for the spring session. Applications will be processed as they are received.

Your
application must be accompanied by a letter of support from a member of your
leadership
team and a non-refundable deposit of $500 (in US$,
drawn from a US bank, and made payable
to Religious Formation
Conference which is applicable to your tuition.
A
limited amount of financial
assistance is available for those who could not participate in the
program
without partial aid. If you need
assistance, your written request for a specified amount
should be sent with the
completed application, and have the stated agreement of the member of
your
leadership team who is writing in support of your application. Decisions about
financial
assistance will be communicated by May 1 or December 1.
